Tanzanie: après le décès de leurs parents des suites du sida, Ahadi et ses deux frères ont été confiés par leurs oncles à différents parents éloignés. Ces oncles ont également détruit la maison familiale en emportant la tôle ondulée pour éviter que les enfants ne soient tentés de revenir.

Ils l’ont fait néanmoins, en cachette, poussés par le besoin de retrouver les lieux de leur ancien foyer. Là, ils ont essayé de survivre tant bien que mal. Une voisine a attiré l’attention de la commune sur leur présence. Un fonctionnaire s’est rendu sur place mais n’a rien remarqué. Un représentant d’une organisation humanitaire a parlé de «quelque chose de tout petit» sous des bananiers, où les enfants vivraient.

Marcellina, la coordinatrice de terre des hommes suisse, en a eu vent et a voulu rencontrer les enfants. Ceux-ci lui ont montré leur minuscule abri sous les arbres. Intriguée, elle leur a demandé comment ils s’y étaient pris pour construire cette petite cabane. De timides qu’ils étaient au début, ils se sont progressivement confiés à Marcellina, mis en confiance par la sympathie et l’intérêt qu’elle leur témoignait. Ils se sont mis à raconter comment ils s’étaient débrouillés pour trouver des perches, ériger une structure et construire des murs avec des branches et des feuilles. Ils lui ont dit qu’ils dormaient à tour de rôle, la hutte étant trop petite pour eux trois. Alors qu’ils narraient leur histoire, leurs visages commençaient de s’illuminer. Bien que ne possédant quasiment rien, ces enfants disposaient d’énormes ressources intérieures, notamment la volonté d’aller de l’avant, la créativité et la ténacité, qui leur avaient permis de revenir vivre sur le lieu où ils avaient grandi avec leurs parents.

La question «Comment avez-vous fait pour vous en sortir jusqu’ici?» («How did you manage to get this far?») est caractéristique de l’«approche orientée vers les solutions». En expliquant de quelle façon ils font face aux difficultés, les clients – en l’occurrence trois enfants – prennent conscience de leurs ressources et capacités.

L’approche orientée vers les solutions est un modèle de coaching développé par Steve de Shazer et Insoo Kim Berg. L’idée de base est que chaque être humain possède des forces et des capacités sur lesquelles il peut s’appuyer pour chercher et trouver des solutions. Plus encore qu’une méthode, cette approche représente une disposition d’esprit grâce à laquelle chaque individu est expert de sa propre vie. Le rôle de l’encadrement consiste à soutenir et guider le client/la cliente dans le processus de prise de conscience et de mobilisation de ses propres ressources et capacités.

Cette histoire est l’un des exemples qu’ont présenté les animatrices/animateurs et les participant-e-s d’un cours de formation de terre des hommes suisse sur les approches orientées vers les solutions en Afrique du Sud. Des expériences faites avec des jeunes de six pays d’Afrique subsaharienne montrent à quel point ce type d’approche est efficace pour fortifier l’esprit et susciter des changements positifs à de nombreux niveaux.

L’approche orientée vers les solutions dans le contexte du VIH/sida sera le sujet de discussion de l’Assemblée annuelle 2009 d’aidsfocus.ch, le 1er décembre à Berne. Cette approche sera également intégrée à la nouvelle version de la «trousse à outils» (toolkit) utilisée pour le travail de mémoire (memory work). Et si tout se passe comme prévu, ce sera le thème d’un atelier mis sur pied par aidsfocus.ch et terre des hommes lors de la Conférence mondiale sur le sida en juillet 2010 à Vienne.

Zum ersten Mal überreichte terre des hommes schweiz im Rahmen des Regionalprogramms Afrika 20 Jugendlichen ein Zertifikat, welches sie als AnwenderInnen für lösungsorientiertes Arbeiten auszeichnet. Die Jugendlichen haben während des einjährigen Pilotkurses «Youth2Youth» gelernt, wie sie Gleichaltrige effektiv unterstützen und beraten können.

Die Zukunft des jungen Mannes aus Zimbabwe scheint düster: HIV-positiv steht er nach zwei Jahren im Gefängnis ohne Job und Perspektiven da. Verzweifelt wendet er sich an die Aids-Anlaufstelle MASO (Midlands AIDS Service Organisation), wo er Dennis, einem der Berater, seine Geschichte erzählt.

Dennis, im selben Alter wie sein Gegenüber, hatte erst vor kurzem eine interne Weiterbildung absolviert. Lösungsorientiertes Arbeiten bedeutet den Fokus auf die Stärken der Ratsuchenden zu richten und nicht auf ihre Probleme. Es bedeutet konsequent nachzufragen, worin sie gut sind. Dabei werden keine Patentrezepte oder Ratschläge verteilt, es gilt vielmehr herauszufinden, was die Betroffenen selber wollen. Nur die Ratsuchenden selbst können sich ihre Ziele setzen, die möglichst konkret, messbar und relevant sein sollen, um in kleinen Schritten umgesetzt werden zu können. Beim Erarbeiten und Finden dieser Ziele können sie von den Beraterinnen und Beratern der Anlaufstellen entscheidend unterstützt werde

Dennis steht diesem Ansatz skeptisch gegenüber. Er befürchtet, dass sich die Betroffenen nicht ernst genommen fühlen, wenn ihre vielfältigen und schwierigen Probleme nicht wie bisher analysiert werden. Sprich, wenn nicht über die Probleme selbst gesprochen und gefragt wird, was schief läuft. Doch Dennis entschliesst sich, dem neuen Ansatz eine Chance zu geben und diesem jungen Mann lösungsorientierte Fragen zu stellen: Was er denn besonders gut könne, will er von seinem jungen Klienten wissen. Zum ersten Mal während ihres Gesprächs schaut ihn der Mann an. Er sei Elektriker. Er hätte sogar im Gefängnis elektrische Arbeiten ausgeführt. Dennis sorgt dafür, dass er zu Werkzeugen kommt und betraut ihn mit elektrischen Arbeiten im Bürogebäude von MASO. Der erste kleine Schritt zur Veränderung ist getan. Bei Dennis und bei dem jungen Mann.

Alle Jugendlichen des Y2Y-Pilotkurses haben das Gelernte an Jugendliche in ihren Organisationen weitergegeben. In den drei einwöchigen Modulen hatten sie Gelegenheit, den lösungsorientierten Ansatz kennenzulernen und eingehend zu üben. Das Programm wurde laufend ihren spezifischen Problemstellungen angepasst. Sie wurden dabei von Theres Steiner, der Kinderpsychiaterin, Buchautorin und Expertin des lösungsorientierten Ansatze begleitet, was für alle Beteiligten ein grosses Glück und Privileg war.

Y2Y ist ein Ausbildungsprogramm, das von terre des hommes schweiz konzipiert, durchgeführt und weiter entwickelt wird. Der Pilotkurs hat bei allen Mitwirkenden eine positive Energie für Veränderungen freigesetzt. Das Konzept dahinter wird deshalb weiter verfolgt, bereits im Oktober startete ein neuer Lehrgang.

Die Aidshilfen in Nordrhein-Westfalen haben im Jahr 2008 das Projekt "Beratung und Test" (BuT) auf den Weg gebracht. BuT steht für den Einsatz des HIV-Tests im Rahmen der niedrigschwelligen HIV-Primärprävention für Männer, die Sex mit Männern haben (MSM). „Die komplexen Anforderungen, die an die Beratung in einem BuT-Angebot gerichtet werden, machten Verfahren und Techniken lösungsorientierter Beratung zu Instrumenten der ersten Wahl. Zunächst stehen die Werte der Aidshilfearbeit für eine unbedingte Akzeptanz eines Ratsuchenden mit seiner Lebenswelt, seinen Entscheidungen und seinem Gestaltungswillen im Vordergrund.“

The cost of HIV medicines is rising all the time, meaning that many people with HIV will not be able to afford life-saving medicines. There is a way to produce new drugs at affordable prices: The UNITAID patent pool for HIV medicines could change this. Join MSF in pushing drug companies to put their patents in the pool. This way drug companies share their drug patents with the pool, so they still get their royalties ….but at the same time other companies can get hold of these patents to make cheaper drugs. Nairobi, 2 November 2009 - A 22-year-old ban on people infected with HIV entering the US was officially lifted on 2 November, with the new rules taking effect in 60 days. AIDS activists have hailed the move as a major coup in the fight against stigma. Samuel Kibanga, national coordinator of the National Forum of People living with HIV/AIDS Networks in Uganda, commented: "This shows that America can now see the reality that people living with HIV are just like any other people, deserving of the right to free movement - the travel ban was discrimination of the highest calibre." Geneva/New York, 31 October 2009 — UNAIDS welcomes President Obama’s announcement of the final rule removing entry restrictions based on HIV status from US policy. The removal of HIV-related travel restrictions in the US overturns a policy that had been in place since 1987. Such restrictions, strongly opposed by UNAIDS, are discriminatory and do not protect public health. “I urge all other countries with such restrictions to take steps to remove them at the earliest,” said United Nations Secretary-General Ban Ki-moon.

Johannesbourg, 27 octobre 2009 - L’annonce récente selon laquelle un vaccin contre le VIH avait montré pour la première fois des effets protecteurs a suscité un grand enthousiasme, jusqu’à ce qu’il apparaisse que les conclusions étaient basées sur les résultats les plus prometteurs de trois analyses différentes des conclusions de l’essai. Quelques semaines plus tard, des chercheurs ayant examiné l’ensemble des données de l’essai, ont dit à la revue Science qu’une analyse fondée uniquement sur des participants ayant reçu l’ensemble des six doses du vaccin aux moments adéquats n’avait pas montré d’effet protecteur significatif au niveau statistique. Geneva, 22 October 2009 - The International AIDS Society (IAS) urged Uganda's political and public health leaders to oppose and reject the Anti-Homosexuality Bill presented last week in Uganda's parliament. If enacted, the bill would dramatically expand existing criminal sanctions on same-sex practices between consenting adults, including authorizing the death penalty in certain circumstances, such as same-sex sexual behaviour by "serial offenders" and people living with HIV.

Nairobi, 15 October 2009 - "With the current financial crisis it is going to be very difficult to achieve the MDGs through official government aid, so innovative methods of financing the fight against these diseases must be sought," said Philippe Douste-Blazy, chair of UNITAID. According to him, the airline levy gives participating developing nations an opportunity to contribute to treatment in their countries rather than depending on handouts from the developed world. Cote d'Ivoire, Niger, Madagascar and Mauritius are applying the airline levy, while Benin, Burkina Faso and Kenya have said they will introduce it. (PlusNews)

Bamako, 14 octobre 2009 - 60 pour cent des enfants qui reçoivent un traitement VIH à l’hôpital Gabriel Touré de Bamako ont perdu un ou deux parents, et ils passent souvent de tuteur en tuteur. Seule la moitié des 935 enfants sous antirétroviraux se rend régulièrement à l’hôpital pour des soins médicaux. Plus de 280 des patients pédiatriques VIH sont classés comme « perdus de vue » et plus de 100 enfants vivant avec le VIH sont morts depuis 2002. Le Mali possède 31 centres de traitement VIH répartis dans le pays et 65 médecins formés pour traiter les enfants atteints du VIH. Les soins pédiatriques VIH sont disponibles depuis 2002. (IRIN)

More than 4 million people in low- and middle-income countries were receiving antiretroviral therapy (ART) at the close of 2008, representing a 36% increase in one year and a ten-fold increase over five years, according to a new report by the WHO, UNICEP and UNAIDS. It highlights other gains, including expanded HIV testing and counselling and improved access to services to prevent HIV transmission from mother to child. "This report shows tremendous progress in the global HIV/AIDS response," said WHO Director-General Margaret Chan. "But we need to do more. At least 5 million people living with HIV still do not have access to life-prolonging treatment and care." (2009)

Strengthening the linkages between sexual and reproductive health and HIV prevention, treatment, care and support is critical to achieving the Millennium Development Goals. This systematic review of the literature on linkages between these two areas - published by WHO, UNFPA, UNAIDS, IPPF and UCSF - was conducted in order to gain a clearer understanding of the effectiveness, optimal circumstances, and best practices for strengthening such links. Women living with HIV and AIDS use language like anyone else, language influenced by culture, class, race and geography. But there is also a whole AIDS language, grown out of AIDS activism, organisations and networks, as well as language which has arisen from more formal responses, such as UNAIDS, governments, research initiatives, donor interests. If we then put on a gender lens how do these words look to HIV positive women? How do they resonate with the way women living with HIV live their daily lives? This Guidance Note from UNAIDS has been developed to provide the UNAIDS Cosponsors and Secretariat with a coordinated human-rights-based approach to promoting universal access to HIV prevention, treatment, care and support in the context of adult sex work. For a number of years, STOP AIDS NOW! and its partners have been investing in stimulating workplace responses among civil society organisations (CSOs) to HIV and AIDS. This report, published by STOP AIDS NOW!, summarises the lessons learnt, and draws on over 40 documents that have been produced detailing the work of STOP AIDS NOW! and its partners. The authors provide a tool for many people to learn from others’ experiences, and so to improve responses to HIV and AIDS in the workplace. (2009)

Worldwide | The World AIDS Campaign has announced the global theme of Universal Access and Human Rights for World AIDS Day 2009. Started on 1st December 1988, World AIDS Day is marked throughout the world and is important in reminding people that HIV has not gone away, and that there is still much to be done to tackle this global epidemic. A focus on universal access is timely because next year will be a milestone for the Millennium Development Goals – encouraging high level review of what has and has not been accomplished in relation to achieving access for all to essential care by 2010.
