En réfléchissant à la problématique «VIH/sida et droits humains» pour cet éditorial, il me revient sans cesse en mémoire l’image de ces jeunes mamans ougandaises réunies, certaines d’entre elles avec leur bébé dans les bras, sur la pelouse de l’église St-Jude à Kampala. Leurs histoires se ressemblent: toutes sont seules responsables de leurs enfants, le père de ceux-ci étant décédé ou les ayant abandonnés. Et la famille élargie ne veut pas de bouches supplémentaires à nourrir. Elles ont trouvé refuge dans un bidonville non loin de là et tentent de survivre comme journalières avec des petits boulots occasionnels tels que nettoyages, garde d’enfants ou petit commerce avec une poignée de charbon et de légumes ou fruits. Durant la journée, leurs enfants sont généralement livrés à eux-mêmes. Souvent, l’argent ainsi gagné ne suffit pas à assurer ne serait-ce qu’un repas par jour, de sorte qu’elles doivent bercer leurs enfants affamés pour qu’ils s’endorment. Beaucoup d’entre eux sont malingres et certaines de ces femmes sont séropositives. En Ouganda, toutes les personnes porteuses du VIH ont en principe accès à un traitement gratuit aux antirétroviraux, mais ces femmes manquent d’argent pour payer leur transport jusqu’à l’hôpital.

Les aspirations de ces femmes sont semblables également: elles rêvent de gagner assez d’argent pour pouvoir se nourrir et nourrir leurs enfants et leur payer l’écolage ou une consultation médicale. Elles désirent ardemment pouvoir offrir une vie meilleure à leurs petits.

Mais à voir les orientations qui sont posées, les perspectives paraissent bien sombres. Les enfants grandissent sans éducation scolaire, sans soins adéquats et sans protection; adolescents ou jeunes adultes, ils ne trouvent pas de travail et tentent leur chance dans la rue, où ils sont livrés à nombre d’influences négatives, y compris au risque de contracter le VIH: le cercle vicieux de la pauvreté et du sida n’en finit alors plus de tourner... Florence Ajok Odoch, dont nous avons pu faire la connaissance lors du symposium d’aidsfocus.ch sur l’avenir de la réponse au sida, s’efforce de trouver avec ces femmes un moyen de sortir de la misère. Elles ont baptisée leur petite organisation «Positive parenting», que l’on pourrait traduire par «éducation positive».

Ce tableau négatif illustre l’importance cruciale de l’approche des droits humains pour la santé et le développement. Le droit à la santé va au-delà de l’exigence de l’accès aux médicaments. Il englobe le droit à des soins appropriés et intègre un large éventail de facteurs socio-économiques qui déterminent les conditions permettant aux femmes, hommes et enfants de mener une vie saine: alimentation, logement, formation, travail, eau potable propre et hygiène adéquate, environnement salubre, égalité entre femmes et hommes et fin de la violence à l’encontre des femmes. Ces déterminants sociaux de la santé – lorsqu’ils sont négatifs – et la violation de droits humains fondamentaux mettent de l’huile sur le feu de l’épidémie de sida.

Bâle | La santé est un droit humain : c'est le fondement souvent oublié de la coopération et de la politique mondiale en matière de santé. Des millions de personnes dans les pays en voie de développement continuent pourtant, chaque année, de mourir de maladies qui pourraient être facilement évitées. Le droit fondamental de l’être humain à la santé est ainsi fondamentalement violé. La situation est pire encore pour les femmes, les minorités et les personnes marginalisées qui n’ont qu’un accès très limité aux soins en raison du mauvais fonctionnement du système de santé. Les droits de l'homme sont inextricablement liés à la propagation et à l'incidence du VIH/Sida sur les individus et les communautés à travers le monde. Le non-respect des droits de l'homme contribue à propager la maladie et à en exacerber l'incidence, dans le même temps que le VIH/Sida sape les progrès réalisés dans la mise en ouvre des droits de l'homme. Ce lien apparaît nettement lorsqu'on considère l'incidence disproportionnée de la maladie sur certains groupes, notamment les femmes, les enfants, et plus particulièrement ceux qui vivent dans la pauvreté, et sa progression. Dans de nombreux pays en développement, le sida et la pauvreté se complètent désormais pour exacerber le pire.

The goal of aidsfocus.ch, the Swiss platform HIV/AIDS and international cooperation, is to "contribute to improving the HIV and AIDS situation in resource-poor countries" (strategy 2009–2011). HIV and AIDS are increasingly threatening to vanish from the development politics agenda. A degree of "AIDS fatigue" would appear to be spreading. This makes it all the more important that aidsfocus.ch and its partner organisations continue to struggle untiringly for the right of all people to health, to comprehensive access to HIV prevention measures, treatment and care, and to support children, women and men affected by HIV and AIDS.

At the aidsfocus.ch conference, held on the 7th of April 2011, participants discussed the challenges and opportunities of linking HIV with sexual and reproductive health and rights, in the light of the current international debate and of concrete experience in Africa and Central Asia. In the countries of the South, Swiss NGOs have been supporting innovative and effective initiatives that provide youth-friendly services concerning HIV and sexual and reproductive health and rights. They address the questions and concerns of young women and men and support them in their efforts to lead a self-determined, healthy life. Die Stiftung Aids und Kind hat viel auf den Weg gebracht. Noch vor 15 Jahren hat man das Thema HIV-positive Kinder in der Schweiz völlig ausgegrenzt, berichtet Linus G. Jauslin im Gespräch mit seiner Nachfolgerin Katharina von Allmen, an die er nach 23 Jahren Tätigkeit für die Stiftung Aids und Kind, die Leitung der Geschäftsstelle übergibt. Jauslin betont, dass neben der Arbeit im Ausland, in Afrika und zukünftig in den GUS-Staaten vor allem die nationale Arbeit ein wichtiges Standbein von Aids und Kind ist.

De notre réaction face à cette réalité dépend la qualité de notre avenir qui sera marqué par la santé, la durabilité et la prospérité ou par les inégalités, la dégradation de l’environnement et les revers économiques. Telle est la position du rapport sur l’État de la population mondiale 2011. «Il faut offrir dès à présent à la jeunesse la possibilité de réaliser son immense potentiel, notamment en investissant dans sa santé et son éducation,» a noté le Dr Osotimehin. « De tels investissements auront un rendement immense en termes de croissance économique et de développement au cours de nombreuses générations à venir.» Il y a des millions d’adolescentes et d’adolescentes qui ne disposent que d’un accès trop limité à l’éducation sexuelle et à l’information sur la façon d’éviter les grossesses ou de se protéger du VIH.

Rio de Janeiro, 20 octobre 2011 - Des hauts représentants gouvernementaux, des experts médicaux et des représentants de la société civile de 120 pays se réunissent à Rio de Janeiro, au Brésil, à l'occasion d'une réunion de trois jours axée sur les conditions sociales, économiques et politiques qui engendrent des inégalités en matière de santé. Organisé par l'Organisation mondiale de la Santé (OMS), la Conférence mondiale sur les déterminants sociaux de la santé constitue une plateforme mondiale pour discuter de thèmes divers tels que le développement du jeune enfant, les soins de santé universels, les protections sociales, le financement équitable, l'égalité des sexes, la responsabilisation politique et la gouvernance mondiale.

Genève, 18 octobre 2011 - La stratégie articule la contribution de l'UNESCO à la stratégie de l'ONUSIDA « Objectif zéro » et appelle à la nécessité d'accélérer l'accès universel à la prévention, au traitement, aux soins et à l'appui en matière de VIH. «Toute l'Organisation est mobilisée aux échelles internationale, régionale et nationale, par le biais d'activités appropriées sur le plan culturel, transformatrices des relations entre les sexes et factuelles. L'approche interdisciplinaire est la puissance de notre signature qui est très adaptée à l'éradication du VIH», a déclaré la directrice générale de l'UNESCO Irina Bokova.

Johannesburg, 20 September 2011 - Health officials in sub-Saharan Africa are finally focusing on non-communicable diseases (NCDs) such as cancer, diabetes and chronic lung disease, having spent much of the past decade concentrating on HIV/AIDS and malaria. However, the responses to HIV and NCDs can take similar approaches, including appointment and medication reminders, transport support, and counselling to support adherence and ongoing behaviour change. Countries are slowly beginning to combine HIV services with chronic disease care. Funding, or lack thereof, however, remains a problem for both HIV/AIDS and NCDs - more so for chronic diseases that lack the high-profile activist campaigns and celebrities found in the AIDS sector. Accra, 31 July 2011- The story of Akua and Aba is a summary of the discrimination people living or suspected to have contracted HIV and AIDS are being subjected. This is often referred to as "the third epidemic" of the disease, the first being the spread of the virus itself, followed by the clinical AIDS. Stigma and discrimination refers to prejudice, negative attitudes, abuse and maltreatment directed at people living with HIV and AIDS (PLWHAs) leading to the victims being shunned by family members, peers and the wider community. The poor treatment of PLWHAs in health institutions and other public facilities tend to erode their fundamental human rights and inflict psychological damage on them. Stigma not only makes it more difficult for people trying to come to terms with HIV and how to manage their illness, but also interferes with attempts to fight the AIDS epidemic worldwide.

The UNAIDS Report focuses on the need of examining and addressing the social determinants of HIV risk, vulnerability and impact in order to achieve universal access to HIV prevention and treatment services. The root causes of health inequities lie in societal conditions, not just in the status of health services. Tackling the social determinants of health requires multi-sectoral diagnosis, and inter-sectoral action. Action must be informed by evidence, rooted in broader development policies and alert to the political dimensions of public health. Building bridges and joining forces with other movements for health and social justice will accelerate progress across the MDGs, optimize efficiency in the use of resources, save and improve more lives, and help to create a world in which Zero inequity becomes a reality. This fully illustrated activity guide is aimed at facilitators and trainers who are starting to work with children affected by HIV/AIDS. Drawing on the experiences of the Alliance, its partners, and other organisations working in the field, it gives some pointers on working with children, including cultural and gender considerations. The guide provides 30 activities for engaging children in group work, as well as fun ideas for ice-breakers and energisers, group work and co-operation, observation, active listening and analytical skills, drama, mime and role play, and painting and drawing. A new book looks back on more than a decade of the life of South African lobby group, the Treatment Action Campaign (TAC), whose brand of activism came to define the world’s fight for HIV treatment and care. The book begins in the dark days of AIDS denialism, moves through the elation of court victories that belatedly ushered in treatment, before settling on the battles ahead – new campaigns that address corrective rape and xenophobia. The strength of the book is the strong link made between milestones and lives - what each decision and protest meant to those living with HIV, their families, their country and ultimately the world's fight for HIV treatment. Worldwide | World AIDS Day this year is about "Getting to Zero. The World AIDS Campaign focus on "Zero AIDS Related Deaths" signifies a push towards greater access to treatment for all. It is a call for governments to act now and a demand that they honour promises like the Abuja declaration and that African Governments at very least hit agreed targets for domestic spending on health and HIV in support of the human right to the best attainable level of health care for all. The decision to go with goal of "Getting to Zero" comes after extensive discussions among people living with HIV, health activists, broader civil society and many others - more than a hundred organisations in all.
