«Qui a dit que tradition et prévention ne rimait pas?» Des mains ornées avec art, dans la plus pure tradition musulmane, tiennent un préservatif. Fascinée, je contemple ce poster apposé dans le bureau de l’Association de lutte contre le sida (ALCS) à Essaouira, ville côtière marocaine. Le message fait mouche.

Un tel franc-parler m’étonne dans un pays musulman. C’est un collaborateur bénévole de l’ALCS qui a conçu ce poster, en s’inspirant de ses conversations avec des adolescents. Malgré les interdits sociaux et religieux, beaucoup de jeunes hommes et femmes ont des relations sexuelles avant le mariage et veulent se protéger contre une infection par le VIH. Le message interpelle directement les gens. De plus, il est une réponse à tous ceux qui prêchent que la tradition protège du VIH/sida et que les jeunes doivent retrouver le chemin de la vertu. Cette affiche a été une grande réussite. Elle a suscité des discussions publiques, et de nombreux préservatifs ont été distribués gratuitement dans des bars et discothèques et à des stands lors des festivals de Marrakech et d’Essaouira.

C’est là un bon exemple de la manière dont des particularités culturelles peuvent être prises en compte et transposées de manière créative. Les conceptions ancrées culturellement sur le «bon» comportement, sur la santé et la maladie influent sur la perception que les gens ont du VIH/sida et, partant, sur l’impact des programmes internationaux de lutte contre l’épidémie. La plate-forme suisse «VIH/sida et coopération internationale» – aidsfocus.ch – se penchera sur cette thématique lors d’une conférence qui se tiendra le 6 mai 2009 à Berne. Nous avons élaboré pour cette journée un très intéressant programme consacré à cette dimension culturelle de la maladie, avec des intervenant-e-s très compétent-e-s et une bonne place réservée à l’échange créatif.

Nous nous réjouissons de discuter avec vous, le 6 mai 2009, des multiples aspects que peuvent revêtir les approches culturelles du travail autour du VIH/sida.

“Sometimes I get the feeling that I talk past people”, says a disillusioned development officer involved in the international response to HIV and AIDS. He is not alone in the feeling expressed in this statement. Many actors in the international AIDS response focus on the biomedical approach, “overlooking” the cultural and social drivers of the epidemic. Conceptions of health and disease, and related beliefs and traditional practices are rooted in culture and have an impact on behaviour and how people cope with HIV and AIDS – and on the success of the response to HIV and AIDS. Culture is “not only the arts and letters, but also modes of life, the fundamental rights of the human being, value systems, traditions and beliefs” (UNESCO).

What does a cultural approach look like? A cultural approach allows prevention and care methods and messages to come from within and be appropriate to different cultures. Creative and artistic projects may be the most visible examples. Cultural appropriate projects include working with religious leaders, elders and healers as well as involving people living with HIV and affected by AIDS.

The aidsfocus.ch conference will explore cultural approaches to HIV prevention, treatment, care and support, based on the experience and knowledge of partner organisations and others.

Objectives of the conference: • To understand the relevance of cultural approaches in the HIV and AIDS response • To share good practice in cultural approaches • To provide motivation for implementation of cultural methods and tools for use in our own field of work

Le projet conjoint UNESCO/ONUSIDA L'approche culturelle de la prévention et du traitement du VIH/sida a été lancé en mai 1998. Son objectif est de stimuler la réflexion et l'action pour une nouvelle approche de la lutte contre cette maladie, basée sur l'application de l’"approche culturelle" dans les stratégies, programmes et projets. Cette stratégie implique les populations dans la lutte contre le VIH/sida, sur la base de leurs références et ressources culturelles.

Afin que les projets, programmes et stratégies qui traitent du VIH/sida soient efficaces, ils doivent toujours remplir les cinq critères suivants : • être culturellement adaptés • promouvoir les droits humains universellement reconnus • être attentifs aux femmes comme aux hommes et promouvoir l’égalité entre les sexes • être adaptés à l’âge de la population visée • inclure les personnes vivant avec le VIH/sida à toutes les phases.

Developing creative cultural approaches to HIV prevention work. This paper from Healthlink Worldwide argues that developing more effective cultural approaches to HIV prevention should be a priority. The predominance of a Western biomedical model in HIV policymaking means that the use of cultural approaches is currently very limited. Where such cultural approaches are being used the emphasis is usually on delivering a message and promoting behaviour change, rather than on facilitating communication and empowerment. The paper recommends that the cultural dimension of the HIV pandemic be addressed through practical action in country strategies and at field level. (2007)

This Key Issue Paper by the Swiss Agency for Development and Cooperation (SDC) emphasizes the cultural approach to prevention as an important tool in the fight against the pandemic. It seeks to establish culture as a determinant of health relevant behaviour and focuses on the use of culture in various forms (theatre, music, dance, traditional medicine, etc.) as a means to communicate and encourage behaviour change for HIV/AIDS prevention (2003).

The Creative Challenge was an action-research project that aimed to improve the effectiveness of HIV/AIDS prevention and coping strategies through valuing local cultures and employing cultural approaches approach to education, information sharing and community dialogue and communications. The project has produced a series of briefings and papers which synthesise and draw on the knowledge, concerns and insight of project partners in four regions as well as on literature and policy developments relating to the role of culture in HIV/AIDS prevention and care. African traditional medicine is often the primary, and frequently the only, accessible health-care option for many people living in sub-Saharan Africa. Early attempts to combine the best of the traditional and the modern systems included a variety of projects that looked at the usefulness of traditional herbal remedies for the treatment of HIV-related illnesses. As part of the best practice collection, UNAIDS has published practical guidelines to help health authorities develop productive relationships between traditional healers and modern biomedical professionals and strengthen the response to AIDS. Bern, 14. Februar 2009 - Im Rahmen der von aidsfocus.ch unterstützten Kampagne von Amnesty International zu den Rechten von Frauen im Kontext von Armut, Gewalt und HIV und Aids in Südafrika weilte Mary Rayner in der Schweiz. Mary Rayner ist die Researcherin von Amnesty International für Südafrika und hat die Situation von jenen HIV-positiven Frauen in ländlichen Gebieten untersucht, die Opfer von Menschenrechtsverletzungen sind.

SolidarMed als Initiantin eines vertikalen Programms? Was hat die Organisation durch das Programm SMART zur Einführung der Aids-Behandlung in acht ländlichen Distrikten des südlichen und östlichen Afrikas gelernt? Blut rettet Leben. Sichere Blutvorräte sind für die Gesundheitsversorgung eines Landes zentral – ganz besonders im Zeitalter von HIV/Aids. The One Million Tests Campaign, organized by AIDS Healthcare Foundation in coalition with 1000+ global partners, has successfully mobilized non-governmental organizations (NGOs), local and national governments, international relief agencies, faith-based organizations, civil society and industry to conduct 1,573,533 Free HIV Tests. The campaign leveraged innovative testing strategies for streamlined and large scale testing – embracing a variety of rapid testing modalities, group pre-test counseling models and easy anti-retroviral treatment (ART) referrals. Le Cap, 16 février 2009 - Vêtu d’un tee-shirt orange portant la mention « HIV positive », Michel Sidibé, le nouveau directeur exécutif du Programme commun des Nations Unies sur le sida, ONUSIDA, a profité d’une visite en Afrique du Sud la semaine dernière pour évoquer ses ambitions pour l’agence. M. Sidibé s’est défini lui-même comme un « incorrigible optimiste » et s’est engagé à faire de l’accès universel aux services VIH/SIDA une priorité. Son objectif est d’éviter 2,6 millions de nouvelles infections et de sauver 1,3 million de vies d’ici la fin 2010. Il a aussi assigné à son agence la tâche de s’assurer que 70 millions de femmes seraient dépistées au VIH et que sept millions d’orphelins recevraient une assistance. (PlusNews)

1. Janvier 2009 - D’après le rapport annuel 2009 du Plan présidentiel d’urgence d’aide à la lutte contre le sida des Etats-Unis, le PEPFAR a dépassé ses objectifs à cinq ans de fourniture de traitements à deux millions de personnes et de soins à dix millions de personnes. Le PEPFAR a soutenu la fourniture de traitements qui sauvent la vie à plus de 2,1 millions d’hommes, de femmes et d’enfants à travers le monde. Il a aussi participé à la fourniture de soins à plus de 10,1 millions de personnes affectées par le VIH, dont plus de 4 millions d’orphelins et d’enfants vulnérables.

Johannesburg, 21 January 2009 - Hopes are high that the change promised by incoming US President Barack Obama will extend to his foreign policy, but for more than two million people living with HIV, mainly in Africa, the changing of the guard at the White House may elicit mixed feelings. The question then, is not whether PEPFAR will continue under Obama's administration, but whether it will continue in the same form. The issue of whether PEPFAR has strengthened health systems by investing in equipment, infrastructure and staff, or weakened them by creating vertical programmes that have drawn manpower and resources away from dealing with other health problems is still up for debate. 1. January 2009 - Inter Press Service (IPS) examined a new emphasis of HIV/AIDS campaigns that focuses on "knowing your epidemic" by analyzing the local HIV/AIDS situation through specific factors that drive the epidemic. The approach helps advocates accurately determine who is HIV-positive and how to address the epidemic successfully with certain populations. IPS cited a Rwandan initiative that involves male partners in the prevention of mother-to-child HIV transmission as an example of the approach. (kaisernetwork)

It affects not only every aspect of the lives of the people concerned, but also considerably impacts the life and work of IC expatriate personnel. St. Gallen | Der 1. Deutsch-Österreichisch-Schweizerische AIDS-Kongress (SÖDAK) vom 24. – 27. Juni 2009 steht unter dem Motto „Prepare for the long run“. Am Kongress nehmen hauptsächlich - aber nicht nur - Spezialistinnen aus dem gesamten deutschsprachigen Raum teil. Die AIDS Epidemie ist noch lange nicht besiegt, weder in Europa noch weltweit. Die Prävention muss ein zentrales Anliegen bleiben. Der Kongress wird sich aber auch internationalen Themen widmen, mit einem speziellen Fokus auf die zunehmende AIDS-Problematik in Ost-Europa.
