Les maladies chroniques, notamment les affections cardio-vasculaires, le diabète et le cancer, atteignent des proportions «épidémiques», avertit l’Organisation mondiale de la santé (OMS). Elles tuent deux fois plus de gens dans le monde que le VIH/sida, la tuberculose et le paludisme confondus.

Pourtant, l’attention du public et des milieux politiques demeure focalisée sur les maladies infectieuses, au premier rang desquelles le VIH/sida – maladies infectieuses qui captent aussi la majeure des fonds, critique l’OMS et d’autres instances.

Cette mise en opposition de différentes catégories de maladies ne fait qu’échauffer les esprits sans rien apporter. Grâce au succès des traitements antirétroviraux, le VIH/sida est devenu lui aussi une maladie chronique qui, comme d’autres maladies chroniques, nécessite une prise en charge suivie et fait peser une lourde charge sur les systèmes de santé, d’autant plus lorsque ceux-ci sont déjà affaiblis.

La question de savoir si les malades du sida doivent être traités de manière préférentielle par rapport aux personnes atteintes de telle ou telle autre maladie chronique, est éthiquement très complexe. Un «traitement d’exception» contre le sida peut aggraver la stigmatisation des personnes séropositives, tout en étant indispensable compte tenu des implications énormes du VIH sur la santé publique et la société en général – implications qui ne peuvent être ignorées. De plus, le non-traitement ou un traitement inadapté peut accentuer la propagation de l’épidémie et ses conséquences. Il n’y a pas de réponse simple.

La manière dont sont abordés les défis de la prise en charge régulière des personnes séropositives dans les pays du Sud, tout particulièrement, peut nous en apprendre beaucoup. «Gestion des maladies chroniques. Expériences issues de la lutte contre l’épidémie de VIH/sida»: ainsi s’intitule l’exposé que donnera Jochen Ehmer, de SolidarMed, lors du Symposium 2009 de la coopération suisse en matière de santé. Cette rencontre, qui se déroulera le 10 novembre à Bâle, sera consacrée aux maladies chroniques dans les pays en développement et les pays émergents. La mortalité liée aux maladies chroniques a dépassé celle des maladies infectieuses dans toutes les régions du monde, sauf en Afrique australe. Le symposium annuel de la coopération suisse en matière de santé, organisé par Medicus Mundi Suisse, est consacré à ce thème. Car les maladies chroniques ne peuvent plus être considérées comme le problème exclusif des pays riches.

En Suisse notamment, les maladies chroniques telles le diabète, le cancer et les affections cardiovasculaires sont considérées par beaucoup comme des maladies du bien-être. Le grand public, et même les milieux de la coopération au développement, méconnaissent le fait que ces maladies touchent également – et de plus en plus – les pays en développement et les pays émergents.

Les affections chroniques représentent un gros défi pour ces pays – où elles font peser une charge toujours plus lourde sur les systèmes de santé nationaux, prioritairement tournés vers la lutte contre des maladies comme le paludisme, la tuberculose et le VIH/sida.

En Asie, en Amérique latine, dans les pays européens en transition et en Afrique urbaine, les maladies chroniques représentent aujourd’hui la première cause de mortalité. Et elles touchent de plus en plus des personnes jeunes – y compris des enfants souffrant de diverses affections chroniques dues à la malnutrition et au manque d’activité physique. Les pays concernés et, plus largement, la politique internationale de la santé et les organisations non gouvernementales actives dans la coopération sanitaire sont confrontés à de gros défis.

Unless…… There is a way to produce new drugs at affordable prices: The UNITAID patent pool for HIV medicines could change this. Join MSF in pushing drug companies to put their patents in the pool. This way drug companies share their drug patents with the pool, so they still get their royalties ….but at the same time other companies can get hold of these patents to make cheaper drugs.

Vor allem beim Besuch der Organisation Batanai, mit dem der fepa seit 10 Jahren verbunden ist, konnten sich eine Schweizer Delegation in Zimbabwe über die wertvolle Arbeit, die hier geleistet wird, überzeugen. Batanai ist ein Netzwerk von Menschen mit HIV/Aids. Als Selbsthilfe-Bewegung gegründet, besteht Batanai heute aus über 700 Selbsthilfegruppen von HIV/Aids-Betroffenen. Ihre Mitglieder unterstützen sich gegenseitig, übernehmen die Pflege von anderen Betroffenen im Dorf und kümmern sich um Waisenkinder. (Reisebericht)

Culture is part of the solution in HIV prevention and the support of people living with HIV and AIDS. Many international reponses to HIV and AIDS fail because of lack of cultural sensitivity and understanding. The aidsfocus.ch conference in May 2009 looked at the complex inter-relationships of culture and HIV and AIDS. Based on experiences and knowledge of partner organisations and others, cultural sensitive approaches to prevention, treatment, care and support of were explored. Johannesbourg, 28 septembre 2009 - Les personnes ayant des partenaires sexuels multiples ne seraient peut-être pas si nombreuses que ce que l’on pensait auparavant, d’après une étude présentée au récent symposium de la recherche sur le sida à Johannesbourg. En général, les gens ont l’impression qu’avoir des partenaires multiples est très courant. Ce phénomène pourrait s’expliquer par le fait que les hommes ont tendance à exagérer le nombre de leurs partenaires. D’autres questions ont montré que la plupart des personnes interrogées – hommes et femmes – ne pensaient pas qu’avoir des partenaires multiples était une pratique acceptable, mais supposaient que les autres le pensaient. Johannesbourg, 7 septembre 2009 - Une initiative visant à aider les éducateurs du monde entier à élaborer des programmes d’éducation sexuelle permettant de limiter le nombre de grossesses non désirées et la propagation des infections sexuellement transmissibles (IST) chez les jeunes, s’enlise dans les controverses. L’UNESCO et le Conseil américain pour l'information et l'éducation sexuelle (SIECUS), qui ont participé à l’élaboration de ce document, l’ont défendu dans des déclarations, affirmant qu’il correspondait à une stratégie fondée sur les faits, visant à réduire le nombre d’infections par le VIH chez les jeunes, et à améliorer leur santé sexuelle et reproductive. September 2009 - AIDS activists around the world have welcomed a new UN General Assembly resolution to create a single agency to promote the rights and wellbeing of women, which they say is good news for women, who are bearing the brunt of the global AIDS pandemic. "We hope the new agency will help with advocacy around women's issues, and will lead to more grass-roots support for HIV-positive women," said Marion Natukunda, project director for the grass-roots Ugandan NGO, Mamas Club. Busia, 14 September 2009 - Discussing the prospect of death is taboo in many African cultures, but a new Ugandan programme is encouraging people living with HIV to secure their families' futures by leaving wills. The programme, part of the Regional Outreach Addressing AIDS project of the NGO, Family Health International, involves teaching will-writing skills, as well as giving older children the skills to manage their parents' property in the event of their death. It also has a “memory book” component, where parents are encouraged to record information, thoughts and messages for their children to read after their death. (PlusNews)

Nairobi, 3 September 2009 - AIDS activists have praised a decision by the Indian patent office to reject patent applications for two life-prolonging antiretroviral (ARV) drugs, allowing Indian manufacturers to keep on making and exporting generic versions. "This is fantastic news that will save countless lives in the developing world - we can now continue to access these two very effective ARVs at an affordable price," James Kamau, coordinator of the Kenya Treatment Access Movement, told. Many developing countries had held off ordering generic tenofovir from Indian companies in case the patent applications were approved. (PlusNews)

This handbook has been produced by International HIV/AIDS Alliance Uganda to help network support agents and other community-based volunteers/providers to be more affective in disseminating standardised HIV and AIDS information at appropriate levels to their audiences. The handbook emphasises that knowledge, skills and the right attitude are needed to identify the psychosocial needs of people infected and affected by HIV and AIDS and address these needs by giving information, counselling and appropriate referrals. The report published by the AIDS, Security and Conflict Initiative (ASCI) provides new evidence and outlines recommendation for actions on the links between security, conflict, peacebuilding and HIV. It is the result of a three year research programme lead by the ASCI. The study indicates that with good policy and appropriate programmes, challenges can be overcome. It recommends 10 ways in which efforts towards peacekeeping, peacebuilding and humanitarian response can integrate HIV issues. (September 2009)
