Chère lectrice, Cher lecteur,

«The benefits of involving men in HIV programmes» – cet article paru sur IRIN Plus News attire mon attention. Notamment parce qu’il y est question de la «prévention de la transmission mère-enfant» (Prevention of Mother to Child Transmission, PMTCT) au Zimbabwe.

Voilà qui est nouveau, pensé-je, les questions liées à l’enfantement ayant été jusqu’ici considérées comme une affaire de femmes essentiellement. A travers l’appellation usuelle «transmission mère-enfant», on attribuait à la seule mère la responsabilité d’une éventuelle infection du nouveau-né par le VIH lors de l’accouchement.

La demande des ONG zimbabwéennes d’associer les hommes à la prévention et au traitement de femmes enceintes est importante, et leurs expériences dans ce domaine sont précieuses. Car comment une femme en train d’accoucher à l’insu du père et de la famille peut-elle prendre le médicament (névirapine) permettant de réduire de moitié le risque de transmission du VIH à son enfant? Comment les mères peuvent-elles recevoir régulièrement, si nécessaire, des médicaments antirétroviraux? Comment résoudre au mieux la question de l’allaitement?

Les questions autour des femmes enceintes et des mères séropositives – et de la transmission verticale du VIH – seront au cœur de la conférence d’aidsfocus.ch du 26 avril 2006. Nous voulons à cette occasion examiner cette problématique dans une perspective globale et aborder des questions aussi bien médicales que socioculturelles et politiques. Avec l’appui d’un groupe d’accompagnement formé de représentant-e-s de six organisations partenaires, nous sommes en train d’affiner le concept et de définir le programme de cette rencontre. Nous vous fournirons dès que possible de plus amples informations à ce sujet.

Le thème de la Journée mondiale de lutte contre le sida sera celui de la responsabilité, prôné toute l’année durant par World AIDS Campaign, qui en assurera la promotion, le 1er décembre, sous le slogan STOP SIDA. Tenez la promesse.

Ce thème de la responsabilité reflète à la fois l’initiative continue de l’équipe World AIDS Campaign au soutien de la Journée mondiale contre le sida et le bilan de la rencontre HIV and AIDS Campaigning and Advocacy, tenue à Londres en février 2005. Ayant tiré les leçons des années précédentes, la campagne a résolu de s’investir davantage, plus tôt dans l’année, pour assurer le succès de la Journée mondiale 2006 contre le sida. L’aspect le plus significatif de cette prochaine journée d’action mondiale tient à la mesure dans laquelle elle repose, cette année, sur l’apport de nombreux et divers partenaires de la société civile.

Le 1er décembre est la Journée mondiale du sida. C’est la journée de la solidarité avec les personnes touchées par le VIH et le sida. Mais elle doit aussi rappeler que pour les personnes atteintes, chaque jour est une journée du sida. En Suisse, cela concerne plus de 20'000 hommes et femmes. Chaque jour, deux nouvelles personnes apprennent qu’elles sont séropositives, et six personnes meurent chaque mois des suites du sida. Il ne peut être question de lever l’alerte et la situation est encore loin d’être normalisée. Le virus peut frapper n’importe qui : femmes et hommes, hétérosexuels et homosexuels, Suisses et étrangers, jeunes et vieux.

Ganze Schweiz | Seit 1988 wird weltweit an jedem 1. Dezember der Welt-Aids-Tag begangen, um Verantwortliche in Politik, Medien, Wirtschaft und Gesellschaft daran zu erinnern, dass Aids und das HI-Virus noch längst nicht besiegt sind. Mit verschiedenen Aktionen in vielen Kantonen wird ab der letzten Novemberwoche der Welt-Aids-Tag vom 1. Dezember begangen. Young People have the Right to Knowledge and Services. Adopting a human rights approach, the organisations participating in the aidsfocus.ch Conference (21 April 2006 in Berne) commit themselves to fully respect the right of young people to comprehensive HIV/AIDS-information and prevention and to address inequality, injustice and gender discrimination. They will continue to advocate with governments and other institutional actors so that human rights are respected in the context of HIV/AIDS prevention, treatment and care.

Luzern, 15. November 2006 - SolidarMed setzt sich seit genau 80 Jahren für die Verbesserung der medizinischen Grundversorgung in ländlichen Gebieten im südlichen Afrika ein. Seit Januar 2005 unterstützt SolidarMed in den vier Ländern Tanzania, Moçambique, Zimbabwe und Lesotho acht langjährige Partnerspitäler beim Aufbau von Kliniken zur medikamentösen Behandlung von HIV/Aids. Eineinhalb Jahre nach der Einführung antiretroviralen Aids-Therapie zieht SolidarMed eine positive Zwischenbilanz. Die 8 Partnerspitäler haben insgesamt über 3’000 registrierte HIV-Patienten, 1’200 von ihnen erhalten eine antiretrovirale Behandlung (ART). SolidarMed verspricht sich von der Einführung der antiretroviralen Aids-Therapie auch eine Verstärkung der begleitenden Präventionsanstrengungen und der Aufklärungsarbeit. Dank der Therapiemöglichkeit verliert Aids an der zuvor fast unbrechbaren Stigmatisierung.

Ruedi Lüthy leitet seit August 2003 in Harare eine ambulante Aids-Klinik, die HIV-positiven Patientinnen und Patienten den Start in ein zweites Leben erlaubt. Einmalig an diesem Projekt ist der Einbezug, und wenn nötig, die Behandlung aller Familienmitglieder. Dies führt zu einer behandlungsrelevanten Therapietreue und bekämpft die Stigmatisierung von HIV in der Gesellschaft. Das neue Buch von Beat Schläpfer berichtet von der konsequenten Aufbauarbeit, von den täglichen Herausforderungen in der Klinik und von bedeutenden Innovationen dieses Hilfsprojekts, mit Originalfotos von Martin Rütschi.

1. novembre 2006 - Ce matin les militants de la Déclaration de Berne et d’Oxfam International ont fait une action devant le siège de Novartis à Bâle afin de dénoncer les tentatives de la compagnie de limiter l’accès aux médicaments génériques à prix abordable en Inde et dans les pays en développement (voir photos). Une action en justice de Novartis en Inde pourrait mettre en danger l’accès aux médicaments dans les pays en développement. Selon l'OMS 30% de la population mondiale n’a toujours pas accès aux médicaments essentiels. 74% des médicaments contre le VIH/sida sont toujours sous monopole (sous brevets) et 77% des Africains n’ont toujours pas accès au traitement du sida. Si plusieurs raisons expliquent cet état de fait, la concurrence des médicaments génériques reste essentielle pour faire baisser les prix et promouvoir l’accès aux médicaments pour tous.

Five years ago, members of the World Trade Organization (WTO) signed a ministerial agreement to ensure that intellectual property rules would no longer obstruct developing countries’ efforts to protect public health. Since then, however, little has changed. Patented medicines continue to be priced out of reach for the world’s poorest people. Trade rules remain a major barrier to accessing affordable versions of patented medicines (generic medicines). The prevalence of debilitating and life-threatening diseases in poor countries is growing, but medicines are simply not available. (Oxfam Report 2006)

This Global AIDS Alliance document describes a framework for a comprehensive response to violence against women and children, including the resources that would be needed, political and financial, for full implementation. It discusses the various arenas and circumstances in which women and children are vulnerable to violence and to infection with HIV, using direct quotes and case studies of women and children’s experiences. It then discusses effective strategies and programmes that are needed to address violence in each situation, illustrating with a series of case studies from South Africa, the Democratic Republic of Congo, Rwanda, Egypt, Liberia and Nicaragua. Building capacity in community HIV/AIDS initiatives - a toolkit produced by the International HIV/AIDS Alliance. It describes the process of community mobilisation: a capacity-building process through which individuals, groups or organisations plan, carry out and evaluate activities on a participatory and sustained basis to improve their health and other needs. The guide has been designed to be used alongside "Tools Together Now! 100 participatory tools to mobilise communities for HIV/AIDS".

This document by the UN Educational Scientific and Cultural Organisation (UNESCO) argues for a harmonised use of language and content that reflects an approach to the HIV and AIDS pandemic that is comprehensive and inclusive, sensitive to the needs and issues of the whole population, but with attention focused especially on key populations vulnerable to HIV.

The objective is to deal with the stigma attached to AIDS, "which is the real impediment to prevention". E.g. “Even though the term HIV/AIDS is widely used and accepted, recent UNAIDS guidelines are promoting the use of HIV where appropriate and AIDS where appropriate. HIV and AIDS are two different issues. The terms HIV and AIDS should be used only when both HIV and AIDS are relevant.” (2006)

Dakar, 17 novembre - Pour la première fois en vingt ans de lutte contre le VIH/SIDA, le Sénégal a décidé de s’attaquer aux difficultés rencontrées par les travailleuses du sexe, les homosexuels et les orphelins, accédant ainsi à de nouveaux financements internationaux.

Entérinée en août dernier par le plan stratégique de lutte contre le sida pour la période 2007-2011, cette réorientation de la stratégie du Sénégal en faveur des groupes les plus vulnérables a permis l’octroi, en novembre, d’une deuxième subvention du Fonds mondial de lutte contre le sida, la tuberculose et le paludisme, d’un montant de 28,9 millions de dollars pour cinq ans. Harare, 10 November 2006 - Prevention of mother-to-child transmission (PMTCT) programmes need to involve men in care and support services, says a Zimbabwean HIV/AIDS organisation. Peter Iliff, medical director of Zvitambo, said his organisation had stepped up efforts to draw men into PMTCT programmes after recognising the benefits of having them on board. When men do play a role, both parties benefit: HIV-positive women are more likely to receive anti-AIDS medication during follow-up visits, avoid breastfeeding their infants, and use condoms; men are more likely to access antiretroviral treatment sooner. Cairo, 8 November 2006 - Hundreds of religious and political leaders in the Arab world on Tuesday called for the region to "break the silence" on HIV, amid reports that cases of the deadly disease are on the rise. The 300 Muslim and Christian leaders from 20 Arab states gathered in Cairo at the Arab League to find ways of breaking the taboo that the disease has in the conservative Arab world. There were more than 67,000 new HIV cases in the Arab world last year. Silence about HIV and AIDS remains the norm in the Arab world, where conservative traditions frown upon discussing sex publicly, and people with the disease are often stigmatized and discriminated.

A project in rural Ecuador worked with a youth group to reduce gender-based power imbalances. Drawings, role plays and focus group discussions helped group members cope with complex issues in innovative and transformative ways. At the participants' request, alcoholism was tackled first. The young men acted out a role play showing how they pressurise each other into drinking in order to maintain a show of appropriate masculinity. They then discussed the implications. In a later session the men used a similar approach to explore the social environment that can lead to casual sex.

Catholics for a Free Choice's Condoms4Life campaign is spearheading an effort to ask Pope Benedict XVI to lift the ban on condoms. In April of this year, sources close to the new pope indicated he had requested that senior theologians and scientists prepare a document on condom use as a means of preventing HIV transmission. Although some Vatican insiders—including Cardinal Alfonso López Trujillo, president of the Pontifical Council on the Family, who claims condoms have minute holes through which the virus is transmitted have insisted there will be no relaxing of the ban, Catholics and non-Catholics around the world remain hopeful that the church will change this policy. According to UNAIDS, approximately 90% of HIV/AIDS victims are cared for at their homes by elderly people. They also enlist in various HIV/AIDS programmes where their close contact with this issue makes their words more persuasive. To help ease the burden of these elderly care-givers, the Viet Nam Women's Union, with sponsorship from the International Elderly Association has carried out the project The Elderly Sympathy Clubs. These clubs aim to improve the health care of elderly people afflicted with HIV/AIDS as well as the patients in their families, through weekly activities like consultation and health care service.

Angry activists living with HIV and AIDS, plan to stage a "die in" in Harare if the government fails to provide them with life-prolonging ARVs in a week's time. Led by the self-styled "General Gunpowder", the group told The Standard in Harare last week they planned to launch what they called a "chimurenga" to force the government to provide ARVs to thousands of mostly poor people living with HIV and Aids. The activists, all with HIV and full-blown Aids, allege the drugs are benefiting mostly "corrupt" government officials and their relatives.

The group belongs to the Zimbabwe National Network of People Living with HIV and Aids (ZNPP+). They said they planned to stage their "die in" at the Ministry of Health and Child Welfare head office in Harare.

1. October 2006 - The South African Government yesterday announced a dramatic reversal of its approach to the country's Aids crisis, promising increased availability of drugs and endorsing the efforts of civic groups battling the disease. "We must take our fight against Aids to a much higher level," the deputy president, Phumzile Mlambo-Ngcuka, told a conference of Aids activists, who until recently had been ignored and even denounced by the government.

Several factors convinced the South African president, Thabo Mbeki, to change his policies. Official statistics show an alarming rise in deaths among South Africans in their 20s, 30s and 40s. Supporters of the ruling African National Congress (ANC) such as trade unions have criticised his policies and the resulting shortages of antiretroviral drugs. Civic groups were joined by 81 leading scientists in demanding the sacking of the health minister who suggested eating beetroot as a cure. And South Africa has fallen behind its neighbours in cutting the infection rate. (The Guardian 28th October 2006)

La matinée de cette rencontre sera consacrée à la présentation du projet année VCT 2007 (projet année conseil et dépistage volontaire 2007).
