Liebe Leserin, lieber Leser,

Kein Baby müsste mit HIV geboren werden, keines an Aids sterben. Was in den reichen Ländern bereits heute gilt, soll bis ins Jahr 2015 für alle gelten. Die Chancen stehen gut, dank dem vereinten Engagement von UNAIDS, UNICEF, dem Global Fund und weiteren internationalen Organisationen, nationaler Regierungen und der Zivilgesellschaft. Die Bilder von unschuldigen Babys, die das HI-Virus quasi als „Geburtsgeschenk“ mitbekommen und in den ersten Lebensjahren an Aids sterben, lassen niemanden kalt.

Die Chancen stehen gut, denn, in den Worten des UNAIDS Direktors Michel Sidibé ausgedrückt: “We know how to get to zero—science has shown the way. The only thing that can stop us now is indecision or a lack of courage. Through strengthened political will and financial resources, we can reach our twin goals of zero new HIV infections among children and zero AIDS-related maternal deaths.”

Die Senkung der Kindersterblichkeit um 40% in den zwanzig Jahren, von 12 Millionen im Jahre 1990 auf 6,9 Millionen im Jahr 2011, ist eine Erfolgsgeschichte der globalen Gesundheit und Entwicklung. Die Fortschritte sind jedoch sehr ungleich verteilt. Besonders kritisch ist es für arme Kinder und Kleinkinder, die bei der Geburt mit HIV infiziert wurden. Aids und damit verbundene Krankheiten sind immer noch eine der Hauptursachen für hohe Kindersterblichkeit in Subsahara-Afrika. Weltweit sind im Jahr 2011 etwa 330 000 Babys mit HIV infiziert worden. Dies sind 24% weniger als 2009. Ohne Behandlung stirbt die Hälfte aller Babys mit HIV noch vor dem zweiten Geburtstag. In Ländern mit hoher HIV-Prävalenz liegt die Kindersterblichkeit im Kontext von Aids zwischen 10% in Mosambik und Sambia und 28% in Südafrika. (UNICEF 2012)

Fortschritte in der Reduktion der Kindersterblichkeit infolge von Aids war möglich geworden dank der raschen Verbesserung des Zugangs zu medizinischer Behandlung zur Prävention der vertikalen HIV-Übertragung, wie die Prävention der Mutter-Kind- Übertragung von HIV (Prevention of mother-to-child transmission of HIV, PMTCT) politisch korrekt heisst. In Ländern mit hohem Einkommen konnte dank umfassender medizinischer Betreuung und medikamentöser Therapie während Schwangerschaft, Geburt und Stillzeit die Übertragung des HI-Virus von der Mutter aufs Kind auf unter 2% gesenkt werden. In Ländern mit geringem und mittlerem Einkommen jedoch erhalten nur 57% der 1,5 Millionen positiven schwangeren Frauen eine antiretrovirale Therapie zur Prävention der vertikalen HIV-Übertragung.

Aids ist die häufigste Todesursache von Frauen im gebärfähigen Alter weltweit (UNAIDS 2012). Das tödliche Risiko für Kinder von positiven Müttern liegt nicht nur darin, dass sie infiziert werden, sondern indirekt auch darin, das HIV-positive Mütter krank werden und nicht mehr in der Lage sind, für ihre Kinder und ihre Familien zu sorgen.

Für die Gesundheit von Mutter und Kind braucht nicht nur einen umfassenden Ansatz zur Prävention der vertikalen HIV-Übertragung, um die Kinder von einer Ansteckung zu schützen: Es braucht auch ein Engagement zur Stärkung von Frauen und jungen Mädchen um HIV-Infektionen vorzubeugen, und von HIV-positiven Frauen zur Verhütung ungewünschter Schwangerschaften, und Gesundheitssysteme, die den Bedürfnissen von Frauen umfassend gerecht werden.

Basel | Die Mutter-Kind-Gesundheit ist einer jener Faktoren, die für die gesellschaftliche Entwicklung in Entwicklungsländern entscheidend sind. Doch jährlich sterben rund 360'000 Frauen während der Schwangerschaft oder der Geburt. 7,6 Millionen Kinder sterben jedes Jahr, bevor sie den 5. Geburtstag feiern können. Das diesjährige Symposium des Netzwerks Medicus Mundi Schweiz wird genauer hinsehen. Was geschieht in der internationalen Politik, um die Situation weiter zu verbessern? UNICEF REPORT: AIDS REMAINS A LEADING CAUSE OF UNDER-FIVE DEATHS DESPITE PROGRESS

A new progress report released by UNICEF titled "Committing to Child Survival: A Promise Renewed" is showing a sharp drop in the estimated number of deaths among children under the age of five worldwide. However, an estimated 3.4 million children under 15 years old were living with HIV in 2011, 91% of them in sub-Saharan Africa. In the same year, about 230,000 died of AIDS-related causes. According to the report, access to antiretroviral therapy (ART) was still low in most countries, with only about 28% of children who needed treatment receiving it in 2011, in contrast to the 57% coverage among adults. UNAIDS REPORT: TOGETHER WE WILL END AIDS

The new UNAIDS report contains the latest data on numbers of new HIV infections, numbers of people receiving antiretroviral treatment, AIDS-related deaths and HIV among children. It highlights new scientific opportunities and social progress which are bringing the world closer to UNAIDS vision of zero new HIV infections, zero discrimination and zero AIDS-related deaths. The report also gives an overview of international and domestic HIV investments. Calling for global solidarity and shared responsibility, the UNAIDS report contains commentaries from global and community leaders as well as people living with and affected by HIV. (July 2012)

PREVENTING MOTHER-TO-CHILD TRANSMISSION OF HIV WITHIN HIV PROPOSALS FUNDED BY THE GLOBAL FUND

PMTCT interventions are guided by a comprehensive strategic approach which includes 4 components: Primary prevention of HIV infection among women of childbearing age; preventing unintended pregnancies among HIV infected women; preventing HIV transmission from a woman living with HIV to her infant, and provision of appropriate treatment, care and support to mothers living with HIV and their children and families. There has been uneven progress to date in implementing these four programming areas. Offering family planning services is critical for component 2 and is an underutilized component in PMTCT services. INTEGRATING HIV WITH REPRODUCTIVE, MATERNAL AND CHILD HEALTH SERVICES

1. October 2012 – Fragmented services create major barriers for women. From a health system perspective, separating out different services for different health issues may be duplicative and highly inefficient. What is the strength of the evidence on integration? Who is most likely to benefit? What are the costs in the long run? And what are the real world challenges and opportunities in integrating services in low-income settings? Last month, in Nairobi, Kenya, these questions were explored at the Integration for Impact conference.

JEDES BABY KANN VOR HIV GESCHÜTZT WERDEN

Der Brennpunkt von SolidarMed aktuell zur vertikalen HIV-Übertragung bringt Beispiele aus der Praxis im südlichen Afrika. Das von SolidarMed unterstützte Dareda-Spital im Norden Tanzanias geniesst in der Bevölkerung grosses Vertrauen, besonders wenn es um die Vorbeugung und Behandlung von HIV geht. Die wartenden Frauen sind selber HIV-positiv. Ihre Kinder sind es nicht. Das von SolidarMed unterstützte Spital ist gut gerüstet im zermürbenden Kampf gegen HIV/Aids. Gemeinsam wurde hier eine HIV-Therapie für Schwangere etabliert, die Babys vor der Übertragung des HI-Virus von der Mutter auf das Kind schützt. SolidarMed setzt diesen Erfolg nun im Norden von Moçambique fort, wo Babys dem Virus nach wie vor ausgeliefert sind. (SolidarMed aktuell, November 2011)

Das Bulletin von Medicus Mundi Schweiz, dem Reader zur aidfocus.ch Fachtagung 2012, bringt aktuelle und anregende Beträge zu Advocacy auf lokaler Ebene im Globalen Süden und in der internationalen und globalen Arena. „Wir können von unseren Südpartnern aus Südafrika, Simbabwe, Tansania und Indien lernen: Sie setzen sich konkret und mutig mit HIV-positiven Menschen und lokalen Gemeinschaften dafür ein, das ihr Recht auf Gesundheit umgesetzt wird, und sie ziehen ihre Regierung zur Rechenschaft, wenn diese ihren Verpflichtungen nicht nachkommt.“ (MMS Bulletin 125, September 2012).

UK: MINISTER RE-DEDICATES DFID TO PRIORITISING HIV AND AIDS

London, 19 October 2012 - Lynne Featherstone MP, the new Parliamentary Under Secretary of State at DFID reflected in her keynote address at the Consortium Annual General Meeting on her time as International Development spokesperson for the Liberal Democrats when she saw for herself the effects of HIV treatment during a visit to a Johannesburg township. The Minister spoke about the relationship between DFID and the UK Consortium, and then moved on to the central message of her address: "I have come here today to emphasize the UK's continued commitment to HIV. I want to make that very, very clear”.

DEUTSCHLAND: BEITRAG AN DEN GLOBALEN FONDS MINDESTENS VERDOPPELN!

Berlin, 17. Oktober 2012 - Aktionsbündnis gegen AIDS und Kindernothilfe starten zweite Phase der Kampagne „Versprechen halten – Aids besiegen". Die Kampagne fordert, den Finanzbeitrag mindestens auf 400 Mio. Euro pro Jahr zu verdoppeln. Dies wäre ein fairer Beitrag, gemessen am tatsächlichen Bedarf des Globalen Fonds’ und an der Wirtschaftskraft Deutschlands. In einem ersten Schritt fand die Parlamentarische Veranstaltung „ Der Globale Fonds – Verstärkt investieren um die HIV/Aids-Epidemie zu überwinden“ am 16.10.2012 statt zu den Herausforderungen des Globalen Fonds und Deutschlands internationalen Verantwortung.

14TH FRANCOPHONIE SUMMIT: SHARED RESPONSIBILITY AND GLOBAL SOLIDARITY

Kinshasa, 13 October 2012 - French President François Hollande confirmed the commitment of France to continue the development of innovative financing through taxes, like the tax on airline tickets, and those on financial transactions which are already in place. This strong commitment to finance the AIDS response reflects the policy of "shared responsibility" advocated by UNAIDS, which calls on international donors to honour their commitments. The speeches delivered by Heads of States at the opening echoed the key messages: “No new HIV Infections, No one denied treatment." (UNAIDS)

INDONESIA IN BOLD MOVE TO OBTAIN CHEAP DRUGS FOR HIV

1. October 2012 - The Indonesian government has quietly made an order which overrides Big Pharma's patents on seven important drugs for HIV and hepatitis B, allowing local drug companies to make cheap and affordable versions for its citizens. The "government use" order was made on Setember 3rd, but with no fanfare and, as yet, no public outcry from the pharmaceutical giants who, in the past, used to defend their patents volubly and aggressively, through the courts as well as diplomatic back-channels. Times have somewhat changed, with much greater public awareness of the toll of treatable diseases and the high price of medicines in developing countries. THE GLOBAL FUND ADOPTS NEW FUNDING MODEL

Johannesburg, 25 September 2012 - The Global Fund to Fight AIDS, Tuberculosis and Malaria has released initial details on its new funding model, which will change the way countries apply for money. The Global Fund board adopted guiding principles of the model at a mid-September board meeting in Geneva. Although the model’s finer aspects are still being developed, key elements include the allocation of funding to country groupings based on disease burden and ability to pay as well as largely foreseen changes to grant application procedures. The new model will group countries by disease burden and World Bank income level, with resulting groups known as “bands”. (PlusNews)

UN THEMATIC COSULTATION: HEALTH IN THE POST-2015 DEVELOPMENT AGENDA

In the current Millennium Development Framework (MDGs) three out of the eight goals directly refer to health conditions (maternal health; child health; AIDS, TB and Malaria). What will be the role of health in the development agenda beyond 2015? The UN is putting in place a series of thematic consultations on the post-2015 development agenda. Health is one of the 9 topics covered. The consultation on health lead by WHO and UNICEF will take place between October 2012 and February 2013. Beyond 2015 appointed the Medicus Mundi International Network to coordinate campaign input in the health consultation.

DEUTSCHLANDS BEITRAG ZUR NACHHALTIGEN EINDÄMMUNG VON HIV. EIN POSITIONSPAPIER DES BMZ

Das vorliegende Papier stellt dar, durch welche Schwerpunktsetzung und Massnahmen Deutschland zur Erreichung der Millenniumsentwicklungsziele beiträgt und in den kommenden Jahren beitragen wird. Die Eindämmung von HIV nimmt in der deutschen Entwicklungszusammenarbeit eine Sonderstellung ein. So verfolgt Deutschland gemeinsam mit anderen Partnern die Gewährleistung des universellen Zugangs (universal access): Bis 2015 soll weltweit allen Menschen, die von HIV betroffen sind, Zugang zu Prävention, Behandlung, Versorgung und Pflege ermöglicht werden. REALIZING SEXUAL AND REPRODUCTIVE RIGHTS. A HUMAN RIGHTS FRAMEWORK

A new briefing published by Amnesty International, explores the linkages between human rights and the improvement of sexual and reproductive health. It assesses the implementation of the ICPD Programme of Action in relation to sexual and reproductive rights. Women’s equality, their ability to make their own decisions freely and without coercion, is central to any effective population and development policy. More than 18 years since the world’s governments adopted the Programme of Action on population and development, it is clearer than ever that the autonomy of women and girls is fundamental to any effective progress. POSITIVE GAINS: PROMOTING GREATER IMPACT ON HEALTH THROUGH HIV AND AIDS PROGRAMMING

The latest policy paper from the UK Consortium on AIDS and International Development focuses on how we can learn from the response to HIV and AIDS to galvanize our approach to global health. Drawing on case studies, the paper explores the ways in which the HIV response has reverberated across health systems, playing a key role in improving access to health services and medicines, highlighting and addressing the shortcomings of national and local health systems, improving quality of care and helping to decrease stigma of marginalised populations. It also considers the role of communities as ‘critical enablers’ in the HIV response. (October 2012)

GOOD PRACTICE GUIDE: INTEGRATION OF HIV AND SEXUAL AND REPRODUCTIVE HEALTH AND RIGHTS

This Good Practice Guide brings together expertise from the HIV/AIDS Alliance global community, current research and other key external materials. This guide includes good practice in four key areas: 1. Community-based and owned approaches which put the most-affected people at the centre of the response; 2. Co-ordinating change at multiple levels to make an impact on HIV and SRH. 3. Joined up approaches to SRHR and HIV, and 4. RESULTS REPORT HIGHLIGHTS ACHIEVEMENTS OF PROGRAMMES AUPPORTED BY THE GLOBAL FUND

The Global Fund results report for 2012, entitled “Strategic Investments for Impact”, provides comprehensive information on the achievements of programmes supported by the Global Fund through June 2012, and discusses some of the strategic issues the Fund is currently addressing. One chapter describes the Global Fund’s contribution towards international targets for HIV, TB and malaria. The report also includes sections on grant performance; support for health and community systems; monitoring and evaluation; the use of new technologies; sources of Global Fund financing; and patterns of Fund disbursements. 08.11.2012 | APIS – 20 JAHRE AIDSPRÄVENTION IM SEXGEWERBE

Bern | Die Aids-Hilfe Schweiz, die regionalen Präventionsverantwortlichen und die Mediatorinnen APiS laden zu einer Kurztagung ein, an der sie über aktuelle Brennpunkte und Entwicklungen in der Prävention bei Sexarbeiterinnen berichten und Fragen beantworten. 2012 arbeiten 13'000 bis 20'000 Frauen in der Schweiz im Sexgewerbe, viele davon Mitgantinnen. 2012 nimmt fast jeder fünfte Mann zwischen 20 und 65 Jahren wenigstens einmal die Dienste einer Sexarbeiterin in Anspruch.

14.11.2012 | HIV/AIDS – MENSCHLICHKEIT IST NICHT TEILBAR

Zürich | Obwohl in den letzten 30 Jahren weltweit enorme Fortschritte gemacht wurden in der Behandlung, der Prävention, der psychologischen Unterstützung sowie in der Begleitung von HIV/AIDS betroffenen Personen, stellt HIV/AIDS nach wie vor eine grosse Herausforderung für die Menschheit dar. Menschen, die mit HIV/AIDS leben, leiden oft an Stigmatisierung und Diskriminierung. Dieses Phänomen ist weltweit und in allen Kulturen festzustellen. Nebst Bundesrat Alain Berset werden engagierte RednerInnen das Thema aus verschiedenen Perspektiven beleuchten. Toumani Diabaté wird als UNAIDS International Goodwill Ambassador und Musiker auftreten.

04.12.2012 | EINE GENERATION IN GEFAHR: PSYCHOSOZIALE UNTERSTÜTZUNG FÜR KINDER IN AFRIKA

Basel | Das diesjährige Symposium der Novartis Foundation für nachhaltige Entwicklung widmet sich thematisch der psychosozialen Unterstützung für Kinder in Afrika. Weltweit haben über 16 Millionen Kinder einen oder beide Elternteile durch HIV und AIDS verloren. Die meisten davon leben in Afrika südlich der Sahara. Die betroffenen Kinder leben in materieller Not, sind sozioökonomisch benachteiligt, gesellschaftlich stigmatisiert, emotional isoliert und psychisch traumatisiert. Dies hat in zahlreichen Ländern weit reichende soziale Folgen und stellt eine Gefährdung für die langfristige Entwicklung dar.

Berlin | Fachtagung des Aktionsbündnis gegen AIDS anlässlich seines 10-jährigen Bestehens. Ausgehend von einer kritischen Bestandsaufnahme sollen erfolgversprechende Handlungsansätze herausgearbeitet werden, mit dem Ziel, die Vision einer Welt ohne HIV und Aids Wirklichkeit werden zu lassen. 25.01.2013 | HIV UND KINDER – ZUGANG ZU MEDIKAMENTEN

Thübingen (D) | Im Mittelpunkt der internationalen Fachtagung der Arbeitsgemeinschaft Tropenpädiatrie vom 25.-27. Januar 2013 in Tübingen stehen die Themen HIV und Kinder sowie der Zugang zu Medikamenten gerade für Kinder in wirtschaftlich armen Ländern. Dazu konnten Dr. Christoph Benn vom Globalen Fonds, Dr. Sabrina Kitaka, HIV-Spezialistin von der Makarere Universität aus Uganda, Samuel Kenneth Ong´ale von der Christian Health Association of Kenya und weitere internationale Gäste gewonnen werden.
